Lembang Agri Gapoktan in Cikidang Village, West Bandung Regency, oversees 207 active members with no membership database, no digital record of seed, fertilizer, and compost procurement, and no transaction evidence for consumers. This study designs, builds, and tests a Farm Management Information System uniting the flow from registration to sales in a single database. The prototyping method runs iteratively through requirements gathering, mock-up construction and refinement, and mock-up testing. Requirements were collected through observation, interviews with the chairperson and the secretary, and review of invoices and harvest reports. The design covers five information system components, with use case and activity diagram modeling alongside data normalization. The system was built on the Laravel framework with a MySQL database, then tested through black box methods using equivalence partitioning and boundary value analysis. The system handles member registration together with land area verification, raw material procurement under tiered approval, planting records, harvest records feeding directly into salable quantity, and sales issuing invoices and notifications by SMS and email. Every test scenario in the final iteration produced output matching the specification.
